What is tension-type headaches?

Tension-type headache is the most common headache type which is characterised by mild-moderate pressing pain on both sides of the head.


How tension-type headaches happen?

The cause of tension-type headache is still unknown, it is currently hypothesised that imbalance in brain chemicals may be the culprit of the headache. There are many contributing factors for tension type headache such as stress, anxiety, depression, drug or alcohol, grinding your teeth, eye strain, poor neck posture, head and neck injuries and hormonal changes.


What are the common symptoms of tension type headache?

  • A “tight band” sensation across the head

  • Dull ache in head

  • Pain usually in the forehead, the back of head and neck

  • Pain that occurs on both sides of the head

  • Headache not aggravated by physical activity

  • Increased sensitivity to light or noise

What are the treatment options for tension-type headache?

  • Physiotherapy to help with posture education, strengthening exercises, spinal manipulation/mobilization, myofascial trigger point massage therapy

  • Medication to help manage pain and other symptoms

  • Psychotherapy to help with stress, anxiety and depression

What are the current evidences about physiotherapy for managing tension-type headache?

A high-quality research by Juan et al, (2016) titled Multimodal manual therapy vs pharmacological care for management of tension type headache: A meta-analysis of randomized trials concluded that multimodal manual therapy provided by physiotherapist is moderately more effective than drug management for short term. Over long term, both multimodal manual therapy and drug management has same efficacy.

Another research conducted by Castien et al (2010) titled Effectiveness of manual therapy for chronic tension-type headache: a pragmatic, randomized, clinical trial compared tension-type headache sufferer who receive 9 sessions of manual therapy from physiotherapy with those who just receive usual care by their general practitioner. It was found that manual therapy is more effective than usual GP care in the short- and longer term (i.e. 6 months) in reducing headache symptoms.
